Patients with established jowling and visible neck laxity are, in most cases, better served by a surgical approach, specifically a deep plane face and neck lift, which repositions the deeper structural layers directly, or in more advanced cases a full facelift, which also addresses displaced volume, excess skin, and the underlying structural changes responsible for facial sagging at the same time. Both are considered the more definitive route for jowling that has progressed beyond what volume or tightening alone can manage.

    The practical difference between these options is not aesthetic preference; it is anatomy. A consultation that properly separates volume loss from tissue laxity is the only reliable way to know which route applies, because treating a structural problem with a volume-based solution tends to produce a limited, short-lived improvement rather than the result a patient is actually looking for.
